Why is my out-of-pocket cost not zero after using points?

Airline loyalty programs only cover the base airfare. Government taxes, airport departure fees, customs inspections, and airline-imposed carrier surcharges (YQ/YR) must still be paid in cash. For long-haul flights, these cash taxes can range from $50 to several hundred dollars.

Do I have to pay resort or destination fees on award hotel stays?

It depends on the loyalty program. World of Hyatt and Hilton Honors waive resort/destination fees completely on award stays. Marriott Bonvoy and IHG One Rewards generally still charge daily resort/destination fees at the front desk even on award bookings.

When is it better to pay cash instead of using points?

Paying cash is recommended when mandatory award taxes are excessive, when your realized CPP is below the conservative threshold for that program (e.g. below 1.2¢ for airlines or 0.6¢ for Marriott), or when cash airfares/hotel rates are extraordinarily cheap and earn significant redeemable and elite status miles.
